New Patient Information

To schedule an appointment, please call (602) 242-6500, or fax your information to (602) 424-6600. Or, you could submit a request for an appointment using the form on the Schedule an Appointment page.

All New Patient appointments are scheduled with more time to allow Dr. Rappoport the opportunity to get to know you and help diagnosis
your symptoms. We ask that you please come in 30 minutes early if your new patient packet isn’t filled out completely.

All the forms needed are attached on this website (see below).

All records and films must either be sent in ahead of time or brought in to your appointment to help us prevent any delays to your treatment. Dr Rappoport prefers the actual films instead of disks. However, some Radiology locations don’t provide films so disks are acceptable in that case. If you find that this is the case. What To Bring To Your First Appointment

  • Insurance card for current coverage
  • Your driver’s license or another valid photo ID
  • All X Rays (film is preferred, or CD)
  • Referral or Authorization
  • A list of any prescription medications and vitamins you’re currently taking
  • A copy of your medical records from your previous doctor or primary care physician; or you can ask your previous doctor to send the health records in advance
  • Your health history, including chronic conditions and illnesses and previous surgeries
  • Plus- don’t forget to print and fill out all the forms listed below, and bring with you

It is also helpful if you check with your Insurance for this information:

  • Your office visit copay, deductible or coinsurance
  • What your insurance covers

 

HIPAA

HIPAA was established to provide federal protection for personal health information. This includes information in medical records, conversations regarding medical treatment and billing information related to the patient’s health. The information on the HIPAA form asks each patient the best way to contact them and if we can leave a message or speak to another person regarding their care. Anytime the information is outdated, please make the office aware so that we can ensure your information is kept current.
